Documents to be Submitted Alongwith the Application Form
Following documents will be enclosed with the application form and the candidates are advised to check the same before sending their form to ACDS :-
  • Admit Card and Attendance Card
  • Medical Fitness Certificates (Certificate No 8).
  • Proof of age as per Std X / XII or Equivalent Examination Mark-sheet/TC
  • Statement of Marks of qualifying examination with aggregate marks
  • Statement of Marks of higher degrees / examinations
  • Appropriate certificate in respect of various categories to avail priority of admission which are as under: -
    1. Certificate No 1 (for dependent of army personnel serving / retired)
    2. Certificate No 2 (for dependents of army personnel killed in action / died during service / disabled in action)
    3. Certificate No 3 (for dependents of Army Medical Crops / AD Crops Officers serving in Air force / Navy Medical Establishment)
    4. Certificate No 4 (for dependent of serving MNS / APS / TA Personnel)
    5. Certificate No 5 (for adopted children of army personnel at least 05 years prior to seeking admission)
    6. Certificate No 6 (for children of widows of army personnel who are born as a result of first marriage and the widow later re-married a civilian)
    7. Certificate No 7 (for step children of widows of army personnel who were born out of wedlock where at least one parent belonged to army)
    8. Certificate No 9 (for unmarried / divorced / widow / legally separated without encumbrances)
    9. Certificate No 10 (Regular study certificate by the concerned institution).
  • Affidavit of Gap in Study.
 
Admit Card
The admit card for the CWAT will be sent to the candidates by ‘Registered Post’ on the address indicated by them on the white envelope provided with the application form. Candidates are requested to affix stamps of the requisite value for the purpose.
The CWAT Admit Card will be issued to the candidate by ACDS Secunderabad.
In case the admit card is not received by the candidate by 15 Apr 2010 she should approach ACDS Secunderabad, stating her full name, address, place of examination as mentioned in the application form and her application form number so as to facilitate the dispatch of the duplicate admit card.
 
Appearing for CWAT in the Examination in the Event of Non Receipt of Army Card
In case a candidate does not receive the admit card prior to the date of examination, she must approach the Officer In charge of examination center (centre as indicated in the application form as first choice) with passport size photograph similar to the photograph forwarded with the application form. Officer In charge examination center may permit the candidate to provisionally appear in the examination. However, evaluation of her answer sheet and admission to the course would in such a situation be subject to the confirmation of her eligibility for CWAT.
